I give the Toronto Junior Golf Tour permission to photograph, videotape, audiotape, film and/or interview either myself, or the person on whose behalf I am giving permission (the "Recordings"), and to publish the Recordings in Toronto Junior Golf Tour publications/material, including marketing and promotional materials, and the Toronto Junior Golf Tour official Web site, both now and in the future. I understand that the Recordings shall constitute the exclusive property of the Toronto Junior Golf Tour and may be reproduced by the City and anyone it has authorized, without compensation or payment to the individual(s) being recorded or any other person. If I am providing this permission on behalf of someone other than myself, I warrant that I have the authority to do so.

The Tour is open to anyone in Toronto age 7-18.

There is a $100 one-time annual registration fee payable at the Don Valley Pro Shop (cash only).
